Welcome to Lumenessence Tattoo!
You agree to the following terms and conditions when booking an appointment with us. Please read carefully.
1. Age and Identification
- Clients must be 18 years or older to receive a tattoo.
- A valid government-issued photo ID is required at the time of your appointment.
2. Health Disclosure
- Clients must disclose any medical conditions, including but not limited to:
- Skin conditions (e.g., eczema, psoriasis).
- Bloodborne illnesses (e.g., Hepatitis, HIV).
- Allergies (e.g., latex, ink pigments).
- We reserve the right to refuse service if health conditions pose a risk.
3. Deposits and Payment
- A non-refundable deposit of $[amount] is required to book an appointment.
- The deposit will be applied toward the total cost of the tattoo.
- Full payment must be made on the day of your appointment. We accept cash, debit, and credit cards.
4. Cancellations and Rescheduling
- Cancellations must be made at least 48 hours in advance to retain your deposit.
- Rescheduling is allowed once per deposit, with at least 24 hours’ notice.
- No-shows or late cancellations will result in forfeiting the deposit.
5. Aftercare and Liability
- Aftercare instructions will be provided after your session.
- We are not liable for complications from improper aftercare or individual healing outcomes.
- Touch-ups are offered within [timeframe, e.g., 30 days] if aftercare guidelines are followed.
6. Artist Rights
- Tattoo designs are the property of the artist until applied.
- We reserve the right to refuse to tattoo designs deemed offensive or inappropriate.
- Clients may not reproduce, copy, or sell tattoo designs without the artist’s consent.
7. Shop Rules
- Clients must arrive on time. Late arrivals beyond 15 minutes may result in rescheduling.
- Please refrain from bringing children or pets to the appointment.
- Only one guest is allowed in the tattoo area.
8. Consent
- By booking an appointment, you acknowledge that you understand and agree to these terms and conditions.
